CONTINUING EDUCATION CREDIT

This activity has been planned and implemented in accordance with the Essential Areas and Policies of the Accreditation Council for Continuing Medical Education (ACCME) through the Joint Sponsorship of Health Forum and the Center for Healthcare Governance. Health Forum is accredited by the ACCME to provide continuing medical education for physicians. Health Forum takes responsibility for the content, quality, and scientific integrity of this CME activity. It designates this continuing medical education activity for 9.0 credit hours in Category 1 of the Physician’s Recognition Award of the American Medical Association.

Each physician should claim only those credit hours that he/she actually spent in the educational activity. Health Forum is authorized to award 9.0 hours of pre-approved Category II (non-ACHE) continuing education credit for this program toward advancement or recertification in the American College of Healthcare Executives. Participants in this program wishing to have the continuing education hours applied toward Category II credit should indicate their attendance when submitting application to ACHE for advancement or recertification. Health Forum is provider approved by the California Board of Registered Nursing, Provider Number 12650 for 9.0 contact hours.
