How to Register
The number of places available for this event is strictly limited and registrations will be accepted on a first-come first-served basis. Due to the interactive structure and core objectives of the program agenda, the CEO and the Board Chair (or Board Chair-Elect) must attend this event together, as a Health System or Hospital Team.
- The cost per Team is $5,000.
- Please click here to print out and mail or fax the completed RSVP with full payment.
- The cancellation deadline is March 1. If the CEO and the Board Chair are unable to attend, requests for refunds must be made in writing by March 1 (less a $1,000 processing fee). Cancellations received after this date are not eligible for a refund.
- Confirmation of registration will be provided in writing to both participants.
